The Chaffee County Health Coalition, created in 2013, is a cross agency collaborative working to identify overlap and duplication of services, and work to improve access to health and wellness in Chaffee County.

In Spring 2015 a task group of this coalition was formed to focus efforts on addressing the growing need for resources for seniors. This group gave rise to Sage Generation. We  are proud to be part of the effort to make it affordable and feasible to age in Chaffee County.

Director = Nick Jones, MS, PT
Nick Jones, a licensed physical therapist, assumed the role of Director of Sage Generation in June 2017, following serving as Board President since its foundation in November 2015.  Nick has lived and worked in Chaffee County for twelve years, providing PT services in Salida’s skilled nursing facility, outpatient, and home health.  During this time, he experienced an extraordinary number of his elderly patients having to move out of this area due to a lack of affordable assisted living and senior housing options.  These were residents who have lived here their entire lives, or the best years of their lives.  Nick is dedicated to improving services and living options for seniors to remain and thrive in this amazing community.

Nick holds a Master’s of Science in Physical Therapy from UCHSC, Denver, and a BA from Humboldt State University.
